Frequency and causes of hypophosphatemia in internal medicine patients

Hubener Lisa , Haas Christian , Harbeck Birgit

Background: Hypophosphatemia is of relevant clinical importance, especially in its severe form. Hypophosphatemia occurs when serum phosphate levels are below 0.84 mmol/l. It can be divided into a mild form (serum phosphate between 0.5 and 0.83 mmol/l), a moderate form (serum phosphate between 0.3 and 0.5 mmol/l) and a severe form (serum phosphate <0.3 mmol/l). Approximately 2.2 to 3.1% of hospitalized patients not requiring intensive care are affected (Brunelli et al., 200...

Assessment of antenatal anxiety and depression in pregnant women with polycystic ovarian syndrome (PCOS)

Floyd Ruairi , Hughes Nessa , O'Sullivan Lisa , Hevey David , Murphy Niamh , Hinds Catherine , Owens Lisa

Background: Women with PCOS experience higher rates of depression and anxiety. There is a paucity of research relating to perinatal mental health in women with PCOS, as well as a lack of evidence-based guidelines for assessment and management of mental disorders specific to pregnant women with PCOS. Limited available evidence suggests PCOS is associated with a higher prevalence of perinatal mental health disorders. However, perinatal guidelines currently do no...

Insulinoma – atypical response to tests and uncommon adverse effects to medical treatment

Kempegowda Punith , Kauser Samina , Shepherd Lisa , Ahmed Mohamed Salih

Background: Insulinoma is the most common cause of hypoglycaemia due to endogenous hyperinsulinemia. However, they can sometimes present with atypical features. Some of the patients can also develop serious adverse effects to medical treatment. We present a case with atypical features and further developed serious adverse effects with medical treatment for insulinoma.Case presentation: A 42-year-old Caucasian female presented with recurr...

Endocrine clinical nurse specialist nurse led clinics – Legal considerations of practice.

Shepherd Lisa , Marland Anne , Austin Rachel , Turner Helen

Introduction: In 2015, a service evaluation of endocrine clinical nurse specialist (CNS) nurse led clinics (NLC) demonstrated they took on a variety of formats to meet local service and patient population requirements. Continuing professional development (CPD) is a fundamental and legal requirement of nursing practice. However, because of the introduction of NMC revalidation in 2016 and recent medicolegal developments such as confidentiality, duty of can...

Investigating the role of cell fate regulator ASCL1 in driving and maintaining lethal neuroendocrine castrate resistant prostate cancer

Bruce Isla , Pang Lisa , Poole Amy , Fraser Jennifer

Neuroendocrine castrate resistant prostate cancer (NE-CRPC) is a lethal CRPC subtype that arises as prostate cancer cells transdifferentiate to neuroendocrine cells, evading the potent selective pressure of androgen deprivation therapies (ADT). Currently, clinicians lack biomarkers to detect NE-CRPC evolution and specific therapies to target it. With ~50% of prostate cancer (PC) patients receiving ADT and NE-CRPC accounting for ~25% of CRPC deaths, CRPC and NE-CRPC are a signi...

Challenges in the management of chronic hypoparathyroidism and severe hypocalcaemia in post thyroidectomy- is there a rationale in using recombinant human parathyroid hormone?

Nijith Lakshmi , Chan Sigmond , Ward Lisa , Bottoms Nicole , Banerjee Ritwik

39-year-old female, with a past medical history of Graves Hyperthyroidism, underwent total thyroidectomy 18 years ago, with resultant hypoparathyroidism, hypocalcaemia and hypothyroidism. Her calcium levels are usually in the range of 1.6-1.8 mmol/l, but there are times when it can go to dangerously low levels (range of 1.4-1.15 mmol/l) causing symptoms, sometimes with ECG changes, requiring hospital admissions and IV calcium replacements. There is no parathyroid activity dete...

Practicality of setting up a Lutetium peptide therapy service

Farhad Mustafa , Chotai Heena , Ganatra Rakesh , Rowley Lisa

Patients with neuroendocrine disease now have the option of having peptide receptor radionuclide therapy (PRRT) for both symptomatic treatment and reducing disease progression. Lutetium (177Lu) oxodotreotide is funded centrally in the UK and several sites have recently begun to provide the treatment for their patients. There are, however, several logistical, operational and regulatory obstacles which must be navigated to ensure the service is both safe and efficient...
